The issue is that the nut on the threaded part of the harness end slides off the threading, basically getting stripped off. The result is the nut falls out of the holder with the nut ending up on the floor and the harness bar falls down on one end.
Several years ago Louet modified one end of the nut on this system so that they were easier to thread onto the threaded part. However, this caused an unexpected consequence of weakening the hold the nut has when engaged on the threaded part.
